肱骨近端骨折手术治疗临床效果观察

目的总结肱骨近端骨折手术治疗的临床效果及手术经验。方法对57例有手术指征的肱骨近端骨折患者分别采用螺钉和普通解剖钢板(普通解剖钢板组,n=38)或肱骨近端锁定钢板(锁定钢板组,n=19)治疗,同时植骨,比较两种手术方式的治疗效果。结果锁定钢板组优14例,良4例,可1例,差0例,优良率94.7%;普通解剖钢板组优15例,良10例,可8例,差5例,优良率65.8%。两组优良率比较差异有统计学意义(χ2=4.27,P<0.05)。结论肱骨近端锁定钢板治疗肱骨近端骨折疗效确切,适应证相对较广。

The Clinical observation of operative treatment of proximal humeral fracture

Objective To summary the clinical result of operative treatmem of proximal humeral fracture. Methods There were 57 cases in this group ,38 cases received internal fixation by plate and screws, the rest 19 cases by proximal humeral locking-plate. Both of the two groups have bone-graft during the operation. After 8 months following-up（4 -12months）, collecting the clinical results of the both groups. Results The locking-plate group was excellent in 14 cases, good in 4 cases ,fair in I cases and poor in 0 cases. The excellent and good rate was 94.7%. The ordinary steel plate group was excellent in 15 cases, good in 10 cases ,fair in 8 cases and poor in 5 cases. The excellent and good rate was 65.8%. The locking-plate group＇s curative effect surpasses the ordinary steel plate group ob- viously. Conclusion The curative effect is exact of proximal humeral locking-plate to treat proxinmal humeral fracture and the indica- tions are relatively wide.
