冠心病伴缺血性J波患者窦性心律震荡现象的临床意义

摘    要:目的探讨冠心病伴缺血性J波患者窦性心律震荡现象(heart rate turbulence,HRT)的临床意义。方法选择2008~2011年于我院进行24小时动态心电图检查患者120例,其中冠心病伴缺血性J波者、冠心病非缺血性J波者及无器质性心脏病者各40例,均检出有室性早搏,计算窦性心律震荡初始(TO)及窦性心律震荡斜率(TS)等指标。结果冠心病伴缺血性J波组TO较无器质性心脏病组高(P〈0.05),TS较无器质性心脏病组低(P〈0.05);与冠心病非缺血性J波组比较,TO及TS也有统计学意义(P〈0.05)。结论冠心病发生缺血性J波时窦性心律震荡现象减弱,更易发生猝死。

Abstract:Objective To study clinical significance of sinus rhythm turbulence with ischemic J wave in patients with coronary heart disease.Methods One hundred and twenty patients were chosen.The patients were checked by 24 hour dynamic electrocardiogram during the period from 2008 to 2011 in our hospital.The patients were divided into 3 groups: coronary heart disease complicated with ischemic J wave(N = 40),coronary heart disease without ischemic J wave(N = 40) and non organic heart disease(N = 40).Ventricular extrasystoles were detected in all the cases.The turbulance onset(TO) index and turbulance slope(TS) index were compared among groups.Results The TO index in group of coronary heart disease with ischemic J wave were higher while TS index lower than that in other two groups(P < 0.05).Conclusions Sinus rhythm turbulence is weaken when coronary heart disease with ischemic J wave.These patients may be more susceptible to sudden death.
