SonoVue超声造影对肝肿瘤的诊断价值

摘    要:目的 探讨超声造影二次谐波成像技术在肝肿瘤诊断中的价值.方法 31例肝肿瘤患者经肘静脉团注SonoVue,采用超声造影二次谐波成像技术,实时观察注射造影剂后病灶增强特征.结果 经超声造影后,23个肝血管瘤动脉相均呈高回声增强,20个表现为周边向心型,2个为中央扩散型,1个为整体型增强特征;20个病灶在门脉相和延迟相仍为高回声增强,2个病灶渐变为等回声增强,1个病灶渐变为低回声增强.4个肝细胞癌动脉相呈高回声整体增强,门脉相1个呈等回声增强,3个呈低回声增强,延迟相均呈低回声增强.4个肝转移癌动脉相呈高回声增强,1个呈不均匀增强,3个呈环状增强,门脉相和延迟相均呈低回声增强.结论 SonoVue超声造影二次谐波成像技术能动态显示病灶增强特征,有助于肝肿瘤的诊断及鉴别诊断.

Abstract:Objective To investigate the value of SonoVue contrast- enhanced ultrasound and second harmonic imaging in diagnosing liver neoplasms. Methods SonoVue was bolus injected via peripheral vein of thity-one patients with liver neoplasms. The technique of contrast-enhanced ultrasound and second harmonic imaging were used to observe the enhancement features of lesions after injection of SonoVue. Results All hemangiomas revealed hyperenhancement during arterial phase and 20 cases of them showed periphery to center enhancement pattern, 2 showed center to periphery enhancement pattern, 1 cases revealed homogeneous enhancement pattern.20 cases showed hyper-enhancement,2 cases lesions showed iso-enhancement and 1 rervealed hypo- enhancementin portal phase and late phase;4 cases hepatoceUular carcinoma showed hyper-enhancement and homogeneous enhancement pattern during arterial phase. 1 revealed isoenhancement and 3 cases showed hypo-enhancement in the portal phase.4 cases revealed hypo-enhancement in late phase;4 cases liver metastasis revealed hyper-enhancement and 1 cases showed inhomogeneous enhancement,3 eases showed ring enhancent in arterial phase. 4 cases revealed hypo-enhancement in the portal phase and late phase. Conclusion The SonoVue contrast-enhanced ultrasound and second harmonic imaging reveals dynamic enhancement features of liver neoplasms, which is helpful to the diagnosis and differential diagnosis.
