多曲方丝弓和平面导板辅助矫治成人前牙深覆的临床对比研究

目的：对直丝弓矫治器结合多曲方丝弓矫治技术（Multiloop Edgewise Arch Wire,MEAW）和结合平面导板打开成人前牙深覆效果进行对比研究。方法：分别应用直丝弓矫治器结合MEAW和平面导板各治疗24例骨性I类前牙深覆成人病例,采用SPSS13.0统计软件对咬合打开的时间及治疗前后病例的双侧X线头颅定位侧位片的测量值分别进行配对t检验。结果：打开覆的时间：MEAW组（3.89±0.24）月,平面导板组（2.15±0.45）月,差异具有显著性P〈0.01。治疗后MEAW组：上、下颌双尖牙与第一磨牙均向远中直立,上、下颌双尖牙伸长,上下前牙唇倾并压低,下颌并未发生明显的顺时针方向旋转;平面导板组：上颌牙齿在角度和长度方面均未发生明显变化,下前牙压低,下颌双尖牙及磨牙伸长,尤以第一磨牙显著,下颌发生顺时针方向旋转。结论：本研究显示直丝弓矫治器结合MEAW技术或结合平面导板均可有效地矫治成人前牙深覆,但临床应用时应注意其各自的适应证。

A clinical contrasfive study on Multiloop Edgewise Arch Wire or Flat Bite Plate in assistant treating adults with anterior deep overbite

Objective This study compared the effects of straight wire appliance with MEAW（the Multiloop Edgewise Arch Wire） or Flat Bite Plate in opening the anterior deep overbite of adults.Methods We applied straight wire appliance with MEAW or Flat Bite Plate to correct 12 adults with skeletal class I the anterior deep overbite respectively.All statistical analysis was performed using the SPSS13.0 statistical package.The time of opening the anterior deep overbite and cephalometric data of cases before and after treatment between groups were compared with t test.Results The mean time of opening the anterior deep overbite in MEAW group was 3.89±0.24 months,it was 2.15±0.45 months in Flat Bite Plate group.The cephalometric analyses before and after treatment in MEAW group show：the mandibular central incisors were inclined labially and depressed.The premolars and first molars were distally erected.Both maxillary and mandibular premolars were protracted.The mandible didn＇t rotate clockwise.In Flat Bite Plate group：the maxillary teeth shows no significant changes in angles and lengths,whereas that the mandibular central incisors were depressed and the mandibular premolars and molars were protracted,especially the mandibular molars.In addition,the mandible rotated clockwise.Conclusion From this study,we conclude that straight wire appliance with MEAW or Flat Bite Plate are both adapted to correct adults with anterior deep overbite.The different compliant cases should be take into account.
