超声检查终末期肾病患者心脏瓣膜钙化的临床价值

目的:探讨心脏超声检查终末期肾病(ESRD)患者心脏瓣膜钙化的临床价值。方法:通过心脏超声检查维持性血液透析ESRD患者36例心脏瓣膜有无钙化及钙化程度,透析开始至少6个月后记录年龄、血钙、血磷、血红蛋白、血清白蛋白(ALB)及C反应蛋白(CRP)等指标。按瓣膜钙化情况分组,比较各组血液学指标。结果:除血红蛋白外,各项观察指标的多组均数比较均有统计学差异。心瓣膜钙化程度与年龄、血钙、血磷、钙磷乘积呈正相关。结论:用本法评估ESRD患者瓣膜有无钙化及钙化程度与钙磷代谢紊乱及微炎症状态有较好相关性,可代表机体内环境紊乱情况。

Clinical value of detection of cardiac valvular calcification on echocardiography in ESRD patients

Objective:To detect the calcification of cardiac valve and its degree on echocardiography in ESRD patients,and evaluate its utility in clinic. Methods: Since the onset of hemodialysis at least 6 months,echocardiography was performed to detect cardiac valvular calcification in 36 ESRD patients undergoing maintenance hemodialysis. Age,serum level of calcium and phosphorus,hemoglobin,serum level of albumin and C reactive protein(CRP) were determined. The patients were divided into 4 groups according to the degree of valvular calcification. Among the groups their difference were compared. Results: All the observed results between groups were of statistical significant,except hemoglobin. Degreee of calcification evealuated by our method had positive correlation with age,calcium,phosphate and calcium-phosphorus product. Decreasing albumin and ascending CRP between calcification group and non-calcification group had statistical significance,but there was no correlation between those changes and degrees of cardiac valvular calcification. Conclusion: In patients with ESRD,diagnosis of cardiac valvular calcification and its evacuated by our method had fine correlation with the disturbance of calcium and phosphate,which could represent the condition of body internal environment.
