腹腔镜手术结合中药外敷在慢性盆腔炎中的治疗价值

目的探讨腹腔镜手术配合中药治疗在慢性盆腔炎中的治疗价值.方法回顾分析我院2000年1月至2002年1月以来应用腹腔镜手术结合中药治疗500例慢性盆腔炎患者,并结合临床预后对治疗效果进行评估.结果术前输卵管通液术提示双侧输卵管通畅者62例,单侧通畅者110例,双侧通而不畅者150例,双侧均不通畅者178例;术后双侧输卵管通畅者102例,单侧通畅者118例,双侧通而不畅者140例,双侧均不通畅者140例.随访142例患者中治愈76例(53.5%),显效 46例(32.6%),好转6例(4.1%),无效14例(9.8%),受孕共24例(16.9%),其中宫内妊娠16例(11.3%),输卵管妊娠8例(5.6%).结论腹腔镜手术结合中药治疗可有效治疗慢性盆腔炎及其引起的输卵管阻塞,迅速矫治输卵管的解剖异常,但对其功能的恢复疗效欠佳.

Therapeutic value of laparoscopic surgery combined with traditional Chinese medicines for chronic pelvic inflammation

Objective To evaluate the clinical value of laparoscopic surgery combined with traditional Chinese medicines for chronic pelvic inflammation. Methods From January 2000 to January 2002, 500 patients with chronic pelvic inflammation received treatment of traditional Chinese medicines after they underwent laparoscopic surgery. The clinical data of these 500 patients were analyzed retrospectively. Results Postoperative hydrotubation showed that bilateral unblocked tubes significantly increased from 62 to 102 (P<0.05), lateral unblocked tubes increased from 110 to 118, partially occluded tubes decreased from 150 to 140, and bilateral occluded tubes significantly decreased from 178 to 140 (P<0.05). The follow-up indicated recovery in 76 patients (53.5%), marked effectiveness in 46 patients (32.6%), and effectiveness in 6 patients (4.1%), but ineffectiveness in 14 patients (9.8%). There were 16 women (11.3%) with normal pregnancy and 8 women (5.6%) with tubal pregnancy in the 142 cases followed up. Conclusion Laparoscopic surgery combined with traditional Chinese medicines is a safe and effective method for the treatment of chronic pelvic inflammation and the occlusion of fallopian tubes, but is not satisfactory for the functional recovery of fallopian tubes.
