多层螺旋CT在骨创伤中的诊断价值

摘    要:目的:探讨多层螺旋CT在骨关节创伤中的诊断价值。材料与方法:29例经CR,普通CT或临床检查可疑有骨折或关节脱位的创伤患者,应用MarconiMx8000型多层CT扫描机进行检查,并行多平面重建图像(MPR),常规容积重建(VR),分析骨折或脱位的显示情况。结果:颅面骨受累15例,脊柱受累5例,四肢关节受累9例;29例中关节脱位10例。MPR矢状位和冠状位重建影像明确诊断27例(93.1%),3D-VR容积重建明确论断0例(71.4%)。MPR显示骨折线的走行和移位更具优势,3D重建更好的显示骨结构的空间改变,骨碎片的移位和关节脱位。结论:多层螺旋CT对骨关节创伤的诊断具有很高应用价值,尤其是对于颅面骨及椎体等不规则骨骨折和关节脱位的诊断更具优势。

Abstract:Objective:To explore the value of multislice spiral computed tomography on trauma of bones and joints.Materials and Methods:Twenty nine patients suspected with fracture or dislocation after trauma were imaged with MSCT,including the techniques of MPR and VR,to show the condition of fracture and dislocation.Cranio facial bones were involved in 15 patients;spines were involved in 5 patients;extremity articulations were involved in 9 patients.Dislocations were found in 10 patients.Results:MPR diagnosed 27 patients accurately,VR diagnosed 20 patients accurately,MPR is more superior in displaying alignment and displacement of fracture than those of other techniques,VR is more accurate in showing space change of bone structures and dislocation of joint.Conclusion:MSCT is a very useful method in the diagnosis of trauma of bones and joints,especially fracture and dislocation of irregular bones.
