糖尿病性心脏病代谢治疗的基本作用

摘    要:胰岛素抵抗、空腹葡萄糖受损、葡萄糖耐量低减以及糖尿病可不断造成心血管病的危险,糖尿病性心脏病有较高的病死率,其主要代谢异常是心肌对碳水化合物的代谢受损,特别是线粒体内丙酮酸氧化作用减少,从而更多的依赖脂肪酸和酮体,健康心脏主要通过摄取葡萄糖转化成丙酮酸,但糖尿病性心脏对丙酮酸氧化能力明显减少,此时在药理上通过脂肪酸氧化的抑制,增加丙酮酸的氧化作用,必定会为糖尿病性心脏病患者带来益处。

Abstract:These condition including insulin resistance, impaired fasting glucose, impaired glucose tolerance, and diabetes mellitus can result in the risk factors of cardiovascular disease. Diabetic heart disease has higher incidence of mortality, whose main metabolic abnormalities in myocardium are impaired carbohydrate metabolism, specifically reduced pyruvate oxidation in the mitochondria,which results in the mere reliance on fatty acids and ketone bodies as fuels. The healthy heart takes up glucose and converts it to pyruvate ; however, diabetic heart has reduced capacity to oxidize pyruvate. Pharmacological inhibition of fatty acid oxidation during diabetic heart disease can increase myocardial pyruvate oxidation and provides clinical benefit to patients with diabetic heart disease.
