急性冠状动脉综合征患者外周血基质金属蛋白酶3水平及临床相关性研究

目的 探讨急性冠状动脉综合征患者外周血基质金属蛋白酶3水平与冠状动脉狭窄严重程度以及基质金属蛋白酶3与心肌酶、肌钙蛋白、血脂等临床指标的关系.方法 比较我院77例急性冠状动脉综合征患者与健康对照者外周血基质金属蛋白酶3水平差异,分析急性冠状动脉综合征患者基质金属蛋白酶3水平与冠状动脉狭窄严重程度;以及基质金属蛋白酶3与心肌酶、肌钙蛋白、血脂等的相关性.采用酶联免疫吸附法测定血清基质金属蛋白酶3水平.结果 基质金属蛋白酶3水平在急性心肌梗死组与稳定型心绞痛组、健康对照组差异有显著性(P＜0.01),在不稳定型心绞痛组与稳定型心绞痛组、健康对照组差异有显著性(P＜0.01),在急性心肌梗死组与不稳定型心绞痛组差异无显著性(P＞0.05),在稳定型心绞痛组与健康对照组差异无显著性(P＞0.05).基质金属蛋白酶3与肌钙蛋白I、肌酸激酶、肌酸激酶同功酶呈正相关;基质金属蛋白酶3与甘油三酯、低密度脂蛋白胆固醇无相关关系.基质金属蛋白酶3水平与冠状动脉造影狭窄严重程度无相关关系(r=0.293,P=0.053).经皮冠状动脉介入治疗的急性心肌梗死患者术前及术后基质金属蛋白酶3水平差异有显著性.结论 急性冠状动脉综合征患者外周血基质金属蛋白酶3水平增高.血清基质金属蛋白酶3水平与冠状动脉狭窄严重程度无关.急性心肌梗死患者血清基质金属蛋白酶3水平与肌钙蛋白I、肌酸激酶、肌酸激酶同功酶呈正相关.
