重症医学科病房89例急性肾功能衰竭临床分析

目的探讨本院危重病医学科(ICU)中急性肾功能衰竭(acute renal failure,ARF)的流行病学、预后及其相关因素。方法收集ICU病房近3年病例中确诊ARF的患者的临床资料,分析流行病学、预后及其相关因素。结果共搜集病例ARF 89例,占同期ICU住院患者的28.34%,本文资料显示近3年来ARF的发病率呈逐年上升趋势。其中医院获得性ARF发生率约65.17%,医院外发生的ARF占34.83%。ICU病房医院获得性ARF患者病死率高达53.45%,并有继续上升的趋势(P<0.01)。结论近3年来,本院ICU病房患者中ARF比率呈增加趋势,ICU病房医院获得性ARF的增加是主要原因,经治疗后其预后却未见明显改善,故应认识到ARF预防的重要性。

Clinical and epidemiological survey in 89 ICU hospitalized patients with acute renal failure

Aim To investigate the epidemiology,prognosis and relative factors of ICU hospitalized patients with acute renal failure.Methods Clinical data of ARF patients in ICU department were collected duiring the last 3 years.The epidemiology,prognosis and relative factors concerning were analyzed.Results During the last 3 years,ARF patients,totally 89 cases,accounts for 28.34% ICU hospitalized patients.The morbility is on an increasing trend during the last 3 years of all ARF patients 65.17% are hospital acquired ARF and the motality of the ICU hospitalized patients reaches up to 34.83%.The incidence of the hospital-acquired ARF constitues 53.45% of all ARF patients and with an significant increase(P<0.01).Conclusion The incidence of the ARF in our ICU department does not decrease and the prognosis is not improved during the three years.The major cause is the HA-ARF.In summary,we should attach much importance to its precaution.
