肌电刺激对周围神经损伤影响的临床研究

摘    要:目的在周围神经损伤手术中应用超强电刺激,探讨电刺激对周围神经损伤后神经再生的促进作用。方法将45例臂丛及周围神经损伤实施松解手术的患者随机分为电刺激组23例、对照组22例。在肌电图仪的持续监测下,电刺激组进行超强电刺激,术后两组同时服用神经营养药物,进行运动疗法,术后随访损伤神经电学变化及其支配肌肉运动功能的恢复情况。结果电刺激组中尺神经损伤、桡神经损伤的复合肌肉动作电位潜伏期改变优于对照组,电刺激组中臂丛神经损伤、正中神经损伤、尺神经损伤的复合肌肉动作电位波幅改变优于对照组,组间差异有统计学意义(P〈0.05)。电刺激组受损神经所支配肌肉运动功能、运动单位恢复优于对照组,组间差异有统计学意义(P〈0.05)。结论周围神经损伤术中应用超强电刺激能有效促进周围神经再生及神经功能恢复。

Abstract:Objective Investigating significance of muscle electrical stimulation of the brachial plexus and peripheral nerve injury supramaximal electrical stimulation to promote nerve regeneration. Method Dividing 51 patients who were given the release surgery into experimental group(26 cases) and control group(25 cases). Giving the supramaximal electrical stimulation to the experimental group in the continuous monitoring of the EMG,,to analysis of the corresponding nerve electrical changes before and after stimulation and sensory and motor function recovery. Results Experimental group before and after electrical stimulation compound muscle action potential latency change(P〈0.05), stimulation is shorter than before stimulation. It has statistical significance with before and after the electrical stimulation compound muscle action potential amplitude changes of brachial plexus injury group, median nerve injury groupand radial nerve injury group which are in experimental group(P〈0.05), experimental group is higher than before control group. The postoperative follow-up that experimental group compared with the control group of nerve dominated the area feeling, movement, motor units are significantly improved, it has statistical significance(P〈0.05). Conclusion Giving the supramaximal electrical stimulation in surgery can effectively promote peripheral nerve regeneration and recovery of neurological function for the injured nerves surviving in continuity.
