游离桡动脉掌浅支皮瓣再造手指末节指腹

摘    要:目的探讨游离桡动脉掌浅支皮瓣再造手指末节指腹的治疗方法及效果。方法回顾性分析2017年1月-2018年6月收治的20例手指末节指腹缺损的患者临床资料,20例均采用游离桡动脉掌浅支皮瓣再造手指末节指腹。经过6~12个月随诊评估游离桡动脉掌浅支皮瓣再造手指末节指腹的治疗效果。结果所有患者皮瓣均成活,供区均Ⅰ期愈合,仅留有线性瘢痕,腕关节屈伸功能较术前未见异常。受区皮瓣质地柔软、外观好、功能佳,带有正中神经掌皮支的皮瓣,术后受区感觉恢复良好。皮瓣两点辨别觉达6~9 mm,平均7 mm。按中华医学会手外科学会手功能评定试用标准评定:优15例,良5例。结论游离桡动脉掌浅支皮瓣具有手术操作简便、皮瓣质地柔软、外观好、功能佳,供区损伤较小且隐蔽等优点,是再造手指末节指腹的理想方法之一。

Free radial artery palmar superficial branch flap for reconstructing the distal digital pulp

Abstract:Objective To investigate the treatment method and effect of free radial artery palmar superficial branch flap on reconstruction of digital pulp.Methods The clinical data of 20 patients with digital pulp defect admitted to our department from January 2017 to June 2018 were reviewed.The free radial artery superficial palmar branch flap was used to reconstruct the digital abdominal pulp.After 6~12 months’follow-up,evaluatied the treatment effect of finger tip reconstructed by free radial artery palmar superficial branch flap.Results All the flaps survived,and all the donor sites healed in one stage,with only linear scar.The flap was soft in texture,good in appearance,and good in function.The flap with the median nerve palmar cutaneous branch had a good sensation of recovery.The two-point discrimination of the flap was 6~9 mm,with an average of 7 mm.According to the trial standard of hand function evaluation of hand surgery society of Chinese medical association,15 cases were excellent and 5 cases were good.Conclusion The free superficial palmar branch radial artery flap has the advantages of simple operation,soft texture,good appearance and good function.The donor area has the advantages of less damage and concealment,which is one of the ideal methods to reconstruct the distal finger.
