VSD治疗手部感染性创面的疗效分析

目的探讨应用负压封闭引流技术(Vacuum Sealing Drainage,VSD)治疗手部感染性创面的临床效果。方法回顾性分析2014年12月-2019年3月收治的皮肤缺损伴感染患者40例。随机分为VSD治疗组(A组)20例,常规换药治疗组(B组)20例。A组给予手术清创,负压引流材料覆盖创面,并给予庆大霉素持续冲洗。B组给予手术清创后,每日换药。两组肉芽组织新鲜后给予植皮治疗。回顾性分析两组平均住院时间、创面愈合时间、创面评分、疼痛程度评分、抗菌药物使用时间、换药次数。结果A组住院时间、创面愈合时间、创面评分、疼痛程度评分、抗菌药物使用时间、换药次数均少于B组,A组疼痛评分低于B组。两组差异有统计学意义(P<0.01)。结论应用VSD治疗手部感染性创面可促进肉芽组织生长,加速创面愈合,减少患者住院时间及抗菌药物使用,减轻患者的痛苦,临床效果良好,值得推广。

Analysis of curative effect of vacuum sealing drainage on infectious wound of hand

Objective To investigate the clinical effect of vacuum sealing drainage on the treatment of infective hand wounds.Methods A retrospective analysis was made on 40 patients with skin defect and infection were admitted to our department from December 2014 to March 2019.They were randomly divided into negative pressure drainage group(group A)of 20 cases and routine dressing change group(group B)of 20 cases.Negative pressure drainage group patients were given surgical debridement,negative pressure drainage material covering on the wound,and gentamicin continuous irrigation.Routine dressing change group was given dressing change every day.Two groups of patients were treated with skin grafting after granulation tissue was fresh.The average hospitalization time,wound healing time,wound score,pain score,antimicrobial use time and dressing change times of the two groups were retrospectively analyzed.Results The hospitalization time,wound healing time,wound score,pain degree score,antimicrobial use time and dressing change times of negative pressure drainage group were less than those of routine dressing change group,and the pain score of negative pressure drainage group was lower than that of routine dressing change group.There was significant difference between the two groups(P<0.05).Conclusion Negative pressure drainage can promote the growth of granulation tissue,accelerate wound healing,reduce hospitalization time,reduce the use of antibiotics and relieve the pain of patients.It has good clinical effect and is worth promoting.
