手背尺侧逆行岛状皮瓣修复小指近中节皮肤缺损

摘    要:目的探讨以尺动脉腕背支为轴心血管的逆行岛状皮瓣修复小指近中节皮肤软组织缺损的临床疗效。方法对2015年4月-2019年5月收治的14例小指近中节皮肤软组织缺损患者,采用以尺动脉腕背支为轴心血管,第5掌骨头颈处为旋转点的逆行岛状皮瓣进行修复,皮瓣供区游离植皮。结果术后皮瓣及植皮均顺利成活,创面均Ⅰ期愈合。随访3~12个月,平均6个月,皮瓣色泽、质地、感觉、外形良好。结论以尺动脉腕背支为轴心血管,第5掌骨头颈处为旋转点的逆行岛状皮瓣修复小指近中节皮肤软组织缺损,手术操作简单,损伤较小,皮瓣成活率高,适宜推广应用。

Repair of skin defect of middle and proximal segment of little finger with the doral ulnar reverse island flap of hand

Abstract:Objective To explore the clinical curative effect of the dorsal carpal branch of ulnar artery as the axis of blood vessel on repairing the soft tissue defect of middle and proximal segments of little finger with its reverse flap of dorso-ulnar aspect.Methods Fourteen patients of skin and soft tissue defects of proximal and middle segment of the little finger from April 2015 to May 2019 were repaired with the dorsal carpal branch of ulnar artery as the axis of blood vessel,the fifth metacarpal neck for the rotation point of retrograde island flap to repair skin and soft tissue defect in the middle and proximal segments of the little finger,skin flap donor site was repaired by free skin graft.Results Postoperative skin flap and skin graft survived,two cases healed in one-stage.The character,color,appearance and the sensation of flap were good,after 3-12 month’s follow-up with an average of six months.Conclusion Using the dorsal carpal branch of ulnar artery as the axis of blood vessel,the fifth metacarpal neck as the rotation point of retrograde island flap to repair skin and soft tissue defect in the middle and proximal segments of the finger has the advantage of simple operation,less injury,high skin flap survival rate,is suitable for promotion.
