窝沟封闭预防儿童第一恒磨牙龋齿效果的meta分析

摘    要:目的评价窝沟封闭预防儿童第一恒磨牙龋齿的效果。方法通过电子检索2009—2019年公开发表的窝沟封闭预防儿童第一恒磨牙龋齿的相关文献,根据循证医学方法建立纳入和排除标准并筛选文献,采用R3.6.1和metafor 2.1软件包进行meta分析。结果共纳入文献24篇,涉及研究牙数122 180颗,其中封闭组牙数72 746颗,对照组49 434颗。窝沟封闭后第1、2、3年随访结果显示,封闭组患龋率均明显低于对照组,3年的合并RR值分别0.12、0.19和0.26。结论窝沟封闭对预防5~9岁儿童第一恒磨牙龋齿效果明显。

Abstract:Objective To evaluate the effect of pit and fissure sealant on caries prevention of first permanent molars in children. Methods We electronically searched literatures published in 2009-2019 and about anticaries effect of pit and fissure sealant in first permanent molars of children. Inclusion and exclusion criteria were established based on the method of evidence-based medicine, and then literatures were screened. R3.6.1 and metafor2.1 package were used for data analysis. Results A total of 24 studies were included in this analysis, and 122,180 teeth were involved, with 72,746 teeth in the pit and fissure sealant group and 49,434 teeth in the control group. After application of pit and fissure sealant, 1-, 2- and 3-year follow-up studies showed that the incidence rates of dental caries were all significantly lower in the pit and fissure sealant group than in the control group, and the pooled relative risk values associated with 1-, 2- and 3-year after application of pit and fissure sealant were 0.12, 0.19 and 0.26, respectively. Conclusions Application of pit and fissure sealant is obviously effective in prevention of dental caries in first permanent molars among children aged 5-9 years.
