术前与术后根除幽门螺杆菌胃癌患者生存率差异的研究

目的研究术前与术后根除幽门螺杆菌(Hp)治疗胃癌患者的生存率差异。方法回顾性选取2014年6月至2016年6月应急总医院收治的胃癌患者60例,所有患者依据根除幽门螺杆菌时间节点不同分为治疗组和对照组,各30例。对照组患者在胃癌切除术前接受根除Hp治疗,治疗组患者胃癌切除术后接受根除Hp治疗。统计分析两组患者的6个月、1年、2年、3年生存情况以及Hp复发情况。结果两组患者术后6个月、1年生存率[100.0%(30/30)、96.7%(29/30)vs.100.0%(30/30)、93.3%(28/30)]比较,差异均无统计学意义(P>0.05)。对照组患者2年、3年生存率[93.3%(28/30)、83.3%(25/30)]均显著高于治疗组[80.0%(24/30)、66.7%(20/30)],差异具有统计学意义(P<0.05)。对照组患者中Hp复发1例,Hp复发率为3.3%(1/30);治疗组患者中复发3例,Hp复发率为10.0%(3/30)。两组患者的Hp复发率比较,差异无统计学意义(P>0.05)。结论胃癌患者术前与术后根除Hp近期疗效相似,但前者更能有效提升胃癌患者2年、3年生存率。

Study of difference of survival rates between preoperative and postoperative eradication of Helicobacter pylori in patients with gastric cancer

Objective To study the difference of survival rates between preoperative and postoperative eradication of Helicobacter pylori in patients with gastric cancer.Methods 60 cases of patients with gastric cancer were selected from June 2014 to June 2016 in Emergency General Hospital were retrospectively selected.All patients were divided into treatment group and control group according to different operation time nodes,30 cases in each group.The patients in the control group received HP eradication before gastrectomy,and those in the treatment group received HP eradication after gastrectomy.The 6-month,1-year,2-year,3-year survival and HP recurrence of the two groups were statistically analyzed.Results The 6-month and 1-year survival rates of the two groups[100.0%(30/30),96.7%(29/30)vs 100.0%(30/30),93.3%(28/30)]had no significant difference(P>0.05).The 2-year and 3-year survival rates of 93.3%(28/30)and 83.3%(25/30)in the control group were significantly higher than those in the treatment group 80.0%(24/30)and 66.7%(20/30),the difference was statistically significant(P<0.05).In the control group,1 case had recurrence of Hp,the recurrence rate was 3.3%(1/30);in the treatment group,3 cases had recurrence,the recurrence rate was 10.0%(3/30).There was no significant difference in the recurrence rate of Hp between the two groups(P>0.05).Conclusion The short-term effect of HP eradication before operation is similar to that after operation,but the former can effectively improve the 2-year and 3-year survival rates of gastric cancer patients.
