妊娠合并子宫肌瘤78例临床分析

目的探讨妊娠合并子宫肌瘤的最佳临床处理方法。方法对78例妊娠合并子宫肌瘤患者(观察组)资料进行回顾性分析,并与无子宫肌瘤的孕产妇(对照组)进行对比,分析妊娠合并子宫肌瘤的患者孕期、分娩期的各种并发症发生情况及处理方法。结果观察组孕期的先兆流产、先兆早产、胎位异常、胎儿宫内发育迟缓(IUGR)、胎膜早破(PROM)发生率高于对照组,差异均有统计学意义(P<0.05);而前置胎盘、妊娠期糖尿病(GDM)发生率与对照组比较差异无统计学意义(P>0.05)。观察组分娩期的脐带绕颈、低体重儿、早产发生率高于对照组,差异均有统计学意义(P<0.05);而胎盘早剥、新生儿窒息、产后出血发生率两组之间差异无统计学意义(P>0.05);观察组剖宫产率为71.8%,远高于对照组的51.4%(P<0.05);有27例在剖宫产术同时对子宫肌瘤进行了剔除,除手术时间延长外,未见明显的出血增加。结论对需要剖宫产手术的孕妇,术中并行子宫肌瘤摘除术是可行的。

Clinical analysis of pregnancy women with uterine fibroids:a report of 78 cases

Objective To explore the best clinical treatment of pregnancy women with uterine fibroids.Methods A retrospective analysis was conducted of data of 78 cases of pregnant women with uterine fibroids(an observed group),compared with the data of 78 health pregnant women(a control group).Results The incidence rates of threatened abortion,sign premature delivery,abnormal fetal position,intrauterine growth retardation(IUGR),and premature rupture of membrane(PROM) in the observed group were higher than those in the control group,and there were significant differences(P<0.05).The incidences of cord around neck,infant of low-birth weight,and premature delivery in the observed group were significantly higher than those in the control group(P<0.05).The cesarean rate was 71.8% in the observed group,which was significantly higher than that in the control group(51.4%).Among 56 cesarean cases,27 cases were treated for removing tumors,and the operation time was lengthened,and the bleeding volume was not significantly higher than that of the others.Conclusions Uterine fibroids enucleation is feasible during the cesarean pregnancy women with uterine fibroids.
