32例急性化脓性胆管炎细菌培养及药敏分析

摘    要:目的了解急性化脓性胆管炎细菌培养及药敏分析结果。方法对32例应用内镜治疗的急性化脓性胆管炎患者术中抽取胆汁进行细菌培养、药敏分析。结果 32例患者中,26例(81.25%)分离出致病菌株64株,其中革兰阴性杆菌34株,占53.12%,共检出革兰阳性球菌30株,占46.88%,大肠埃希菌对亚胺培南、头孢他定、头孢曲松的敏感率分别为93%、87%、82%;肺炎克雷伯菌对亚胺培南、阿米卡星、头孢曲松的敏感率为100%、85%、81%;肠杆菌药物的敏感性较差;铜绿假单胞菌虽然对亚胺培南、阿米卡星敏感性较高。革兰阳性球菌中主要为粪肠球菌,对万古霉素、亚胺培南的敏感率为93%、88%。葡萄球菌属仅对万古霉素敏感。结论实际工作应依据细菌培养及药敏结果调整抗生素的使用,避免滥用抗生素减少耐药菌株的出现。

Bacterial culture of 32 patients with acute suppurative cholangitis

Abstract:Objective To understand the status of bacterial culture and sensitivity of patients with acute suppurative cholangitis. Methods A total of 32 acute suppurative cholangitis patients who received endoscopic treatment underwent the bile collection for bacterial culture and sensitivity analysis.Results Totally 64 strains of pathogenic bacteria were isolated from 26 cases(81.25%) among the 32 patients,and the bacteria included gram-negative bacilli(34,53.12%) and Gram-positive cocci(30,46.88%).The sensitivity rates of Escherichia bacteria to imipenem,ceftazidime and ceftriaxone were 93%,87% and 82% respectively;the sensitivity rates of Klebsiella pneumoniae to imipenem,amikacin and ceftriaxone were 100%,85% and 81%respectively.Enterobacteriaceae was less sensitive to the drugs.Pseudomonas aeruginosa was higher sensitive to imipenem and amikacin.The sensitivity rates of Gram-positive cocci,mainly Enterococcus faecalis,were 93% and 88% to vancomycin and imipenem respectively.Staphylococcus aureus was only sensitive to vancomycin.Conclusions In the practice,physicians should adjust the use of antibiotics based on bacterial culture and sensitivity results,so as to avoid the abuse of antibiotics to reduce the emergence of resistant strains.
