经口胆道子母镜临床应用

目的总结经口胆道子母镜的临床应用价值。方法应用经口胆道子母镜对４０例患者进行胆道检查和治疗。结果子母镜胆管检查成功率达９５％；其中１例左肝内胆管结石采用子母镜取石网篮取出结石，３例胆总管巨大结石（＞２ｃｍ）配合液电碎石（ＥＨＬ）治疗，首例因未能解决碎石后视野不清的问题，中途放弃。后２例置入鼻胆引流管（ＥＮＢＤ）冲洗，保证了ＥＨＬ在有水的环境中进行，碎石、取石成功。结论子母镜尽管操作复杂，但其子镜可经口在直视下观察胆、胰管粘膜早期病变，并能作碎石等治疗，是一种较安全、有效的诊疗方法。

Clinical application of peroral biliary mother babyscope

Objective The clinical value of peroral biliary mother babyscope was verified.Methods In this article 40 patients underwent cholangiopancreatoscopy with the mother babyscope.Results The rate of success attained 95%.A left intrahepatic biliary stone was successfully extracted with basket.Electrohydraulic lithotripsy(EHL) was tried through the scope in 3 cases with big choledocholithes(>2.0cm in diameter).Stone crush and removal was succeeded in two of them with the help of placing a nasobiliary tube for continuous biliary irrigation to ensure good view for the procedure.Conclusion By preoral mother babyscope the mucosal lesion can be viewed and stone removed under direct vision,however the manipulation seems troublesome.It is of considerable value in clinical application.
