人工机械通气在耳鼻喉科患者呼吸困难中的应用研究

摘    要:目的探讨人工机械通气在耳鼻喉科患者呼吸困难中的应用。方法选取在本院耳鼻喉科接受人工机械通气的34例患者为研究对象,另外随机选取34例耳鼻喉科呼吸困难患者采用面罩给氧作为对照,比较两组患者的差异性情况。结果两组患者在治疗后15 min以及在腹胀、肺气肿、气漏、右心衰竭等并发症方面比较无明显差异性(P>0.05),但是在治疗后30min、治疗后60 min以及在总并发症发生率和平均症状缓解时间、住ICU天数、气管切开、平均治疗时间上比较有明显的差异性;另外在PH、PaCO2、PaO2、HCO3-、乳酸、呼吸频率、心率上比较有明显差异性,且治疗前后也有明显的差异性,P<0.05,均具有统计学意义。结论人工机械通气在治疗耳鼻喉科患者呼吸困难中作用明显,可降低气管切开率和提高抢救成功率。

Abstract:Objective To investigate the application of artificial mechanical ventilation on otolaryngological patients with dyspnea.Methods 34 otolaryngological patients treated with artificial mechanical ventilation in our hospital were selected as the research group,and 34 otolaryngological patients with dyspnea received mask oxygen inhalation were selected as the control group.The differences between the two groups were compared.Results There was no clear difference of complications between the two groups 15min after the treatment,and the complications included abdominal distension,emphysema,air leakage and right heart failure(P〈0.05).However,the difference of the two groups showed statistical significance 30min after the treatment.The improvement of incidence of complications,duration of hospital stay,symptom relieve time,levels of PH,PaCO2,PaO2,and HCO3,and heart and breathe rate was more pronounced in the research group than in the control group(P〈0.05).Conclusion Artificial mechanical ventilation on otolaryngological patients with dyspnea not only has evident effect,but also can reduce tracheotomy rate and improve salvage success rate.
