胸膜腔注入尿激酶治疗结核性多房性胸腔积液价值

摘    要:目的探讨超声引导胸膜腔内注射尿激酶治疗结核性多房性胸腔积液的临床应用价值。方法将40例结核性多房性胸腔积液患者随机分为研究组和对照组。研究组在超声引导定位下向胸腔内穿刺抽液后,注射尿激酶10万U治疗;对照组给予胸膜腔穿刺抽液治疗。结果 1.研究组患者胸腔积液消失时间要短于对照组,胸膜肥厚度小于对照组,肺功能中FEV1%、FVC%高于对照组,其差异均具有统计学意义(P<0.05);2.研究组患者总有效率为95%;对照组患者总有效率65%,差异具有统计学意义(P<0.05)。结论超声引导胸膜腔内注射尿激酶治疗结核性多房性胸腔积液是一种安全、可靠、有效的治疗方法,值得临床推广应用。

Abstract:Objective To investigate the application value of ultrasound-guided pleural cavity injection of urokinase in the treatment of multilocular pleural effusion.Methods 40 patients with multilocular tuberculous pleural effusion were randomly divided into the study group and the control group.The study group was treated with ultrasound-guided positioning to puncture fluid within the pleural injection of urokinase 100,000 U treatment,and the control group was given the pleural cavity puncture fluid therapy.Results The disappearance time of pleural effusion was shorter in the study group than in the control group.The thickness of pleura was less in the study group than in the control group.The levels of FEV1 and FVC% were higher in the study group than in the control group.The total effective rate was 95% and 65% respectively in the study group and in the control group with significant difference(P〈0.05).Conclusion Ultrasound-guided pleural cavity injection of urokinase is a safe,reliable,and effective method in the treatment of multilocular pleural effusion,which is worthy of widely clinical application.
