摘    要:目的研究血细胞比容(Hct)时创伤并发急性肺损伤(Au)患者氧代谢的影响。方法对26例创伤并发ALI患者在Hct不同水平通过Swan—Ganz漂浮导管监测其氧代谢的变化情况。结果随着创伤并发Au患者Hct的增加,血液黏稠度逐渐增加,机体氧供(D02)逐渐增加,而氧摄取率则逐渐降低,氧耗始终处于相对稳定的水平。结论Hct的增加会引起机体氧供的增加,氧摄取率的下降,这可能是由于Hct〉30%增加了血液黏稠度,导致血流缓慢,从而抑制细胞氧运转所致。

Abstract:Objective To investigate the effects of hematocrit on oxygen metabolism in traumatic patients with acute lung injury(ALI).Methods The hemodynamics and the oxygen metabolism parameters of a total of 26 traumatic patients with ALI were monitored through a Swan-Ganz tube at Hct levels ranging from 25% to 50%.Results Systemic DO_2 increased steadily and significantly with increased Hct,but systemic O_2 extraction ratio(ERO_2) decreased significantly.Systemic VO_2 was not affected by the changes in Hct.Blood viscosity increased progressively with increasing Hct. Conclusions A progressive increase in Hct causes a corresponding increase in systemic DO_2,and a decrease in ERO_2.Hct>30% may in fact cause a decrease in blood flow rate and increase in blood viscosity,which may inhibit cellular oxygen transfer.
