非插管静吸复合全麻用于小儿腹股沟斜疝腹腔镜手术麻醉的临床观察

目的观察非插管静吸复合全麻在小儿腹股沟斜疝腹腔镜手术中的麻醉效果、循环血气变化和术后苏醒过程。方法60例行单侧腹股沟斜疝腹腔镜手术的患儿，随机分为插管全麻组（对照组30例）和非插管静吸复合全麻纽（观察组30例），观察手术时间、气腹效果、清醒时间，连续监测平均动脉压（MAP）、心率（HR）、血氧饱和度（SpO2）、呼气末二氧化碳分压（PetCO2）。结果两组气腹效果优良率均为100％；两组患儿手术时间（min）差异无显著性（P〉0．05）；两组患儿苏醒时间（min）观察组短于对照组，组间比较差异有显著性（P〈0．05）。两组患儿气腹后的MAP、HR、PetCO2均较气腹前显著升高（P〈0．05），对照组和观察组间比较差异无显著性（P〉0．05）。结论与插管全麻相比，非插管静吸复合全麻用于小儿腹股沟斜疝腹腔镜手术同样是安全有效的，但非插管静吸复合全麻适用于手术时间短的腹腔镜手术。在手术时间〉1h的手术，以气管插管控制呼吸为宜。
