膝关节镜下离子冷凝刀的临床应用价值

目的 报道关节镜监视下离子冷凝刀临床应用的实践经验及优势特点。方法 将72例病例随机分成2组,对照组和试验组,每组36例,所有病例均为半月板病变、滑膜病变和骨关节炎。对照组病例的治疗为在关节镜下,全面探察确诊后,应用常规手术器械对病变组织进行剪切、清理和刨削、钻孔减压,而试验组病例是在关节镜监视下,应用离子冷凝刀必要时结合常规器械,对病变组织进行清理、刨削和钻孔减压的基础上,予以切割和消融,术后均配合早期功能练习等康复治疗。结果 经6～24个月有效随访,所有的病例均无感染和再发积液等严重并发症发生,关节功能恢复良好。两组术后疗效比较,在术后症状缓解时间及程度、术后关节液血液成分含量分析、术后病人卧床时间及术后相同时间内行走距离等几方面,试验组获得更佳疗效。结论 关节镜下离子冷凝刀技术的应用比镜下应用常规剪切、刨削等器械对膝关节疾患进行治疗,具有创伤小、出血少、功能恢复快、疗效好、并发症少等优点,可缩短疗程,降低医疗费用。

Arthrowand technique under arthroscope in diagnosis and treatment of knee disease

Objective To summarize the clinical experience in diagnosis and treatment of knee disease by arthrowand technique under arthroscope. Method Seventy-two patients were devided randomly into control and test groups (each 36 patients). The patients suffered from menisectomy, synovitis and arthritis. The control group confirmed and treatd by knee arthroscope, was subjected to debridement, cleansing and hole drillins for reduction of pressure. The test group was treated by arthrowand under arthroscope. All patients received rehabilitaton therapy. Results The patients developed no infection or complication. Better therapeutic results were obtained after follow up of 6 to 24 months. Conclusion Arthrowand technique under arthroscope forthe diagnosis and treatment of knee diseases has the advantages of less injury, bleeding, and complication but quick recovery.
