无白内障手术经验学习超声乳化手术快速过渡方法及其探讨

目的 探讨无白内障手术经验的医师直接学习超乳手术过程中早期手术技巧及其并发症,并分享经验.方法 经白内障超声乳化理论、手术助手学习,选择合适病例,顺利完成53例（59眼）白内障超声乳化手术.结果 53例（59眼）手术均能顺利完成,人工晶状体植入率（100％）,术后第1天最佳矫正视力较术前提高者46眼（77.97％）,第7天最佳矫正视力较术前提高者57眼（96.61％）,术中撕囊发生滑向边缘7眼（11.86％）,后囊膜破裂3眼（5.08％）,悬韧带部分断离2眼（3.39％）,虹膜损伤脱色素2眼（3.39％）.术后第1天角膜水肿20眼（33.90％）,均于术后2～7天内消退.结论 经过白内障超声乳化理论及助手培训学习,规避手术风险,无白内障手术经验的医生在上级医师指导下能够在短期内掌握超声乳化手术技术.

Investigation of quickly transition of phacoemulsication cataract surgeries at early learning stage of surgeons without experience of cataract surgeries

Objetive To investigate the effect of operative skills and the related complications of phacoemulsication cataract surgeries at early learning stage of surgeons without experience of cataract surgeries. Methods Learning included theoretical study, and case selections. Phacoemusification were performed in the 53 cataract patients （ 59 eyes） . Results Fifty-nine cataract surgeries were done by author independently and intraocular lens were implanted during each surgery. Compared with preoperative visual acuity, 46 eyes （77.97%） had a better visual acuity one day after surgery, and 57 eyes （ 96.61% ） had a better visual acuity seven days after surgery. The main complications included anterior capsule sliding off to the equator during capsulorhexis （7 eyes,ll.86%）,posterior capsule break （3 eyes, 5.08%）, iris injury （ 2 eyes, 3.39%）, partial dehiscence of the zonule（2 eyes, 3.39%）, cornea edema （20 eyes, 33.90%）. Cornea edema disappeared during 2 to 7days after operation. Conehzsions Phaeoemusification can be a safe procedure even during the early learning stage for a surgeon without experience of cataract surgery. By means of learning step by step, training followed an experienced teacher, this technique can be mastered in a short term.
