| 摘 要： | 目的]探讨多中心骨巨细胞瘤的临床、影像和组织病理学特点.方法]9例20处肿瘤均经组织病理学证实为骨巨细胞瘤,影像学分析肿瘤发生部位和特点,组织病理学确定组织学形态特点,临床资料及随访确定肿瘤治疗方法和效果.结果]女性多于男性,女:男为2:1.3例＜20岁,平均21岁.大部分发生于长骨,膝关节上下最常见.位于长骨的肿瘤影像学上表现为干骺端膨胀性、溶骨性破坏并延伸至骺端,3例病人3处局限于干骺端,不侵犯骨骺,1例出现成骨性肿瘤区.组织病理学上部分肿瘤区可出现纤维组织细胞、反应性骨形成及动脉瘤样骨囊肿.手术采用刮除植骨或广泛切除,3例复发,1例死于肺转移.结论]多中心骨巨细胞瘤多发于年轻病人.常见于膝关节周围,部分肿瘤局限于干骺端.各处肿瘤之间是相互独立的而非由初次发生的肿瘤转移而来.肿瘤是否复发与外科手术方式有关.
