胃癌及癌前病变中幽门螺杆菌与p53基因表达关系的研究

目的为研究胃癌及癌前病变中幽门螺杆菌(HP)与p53基因表达的关系,探讨HP致癌的可能机理.方法应用免疫组化S-P法和甲苯胺蓝染色法.结果胃癌组HP阳性检出率为65%(26/40),明显高于慢性浅表性胃炎组32.5%(13/40),两者比较有显著差异性(P＜0.01);HP阳性胃癌组中p53阳性表达率为73.1%(19/26),高于HP阴性胃癌组p53阳性表达率50%(7/14),两者比较有显著性差异性(P＜0.05),HP阳性胃癌组中p53基因阳性表达率与胃癌的分化程度、浸润深度、淋巴结转移有关.结论胃癌及癌前病变与HP感染有密切关系,可能是胃癌发展过程的主要诱发因子.其作用机理可能是通过HP产生损伤因素如细胞毒素等作用,减弱了胃粘膜的保护因素,使组织损伤,胃上皮细胞增殖加速,导致基因突变,使胃粘膜上皮细胞恶性转化.另外HP可能在p53基因突变中起促进作用,使其失去正常抑癌功能,导致胃上皮细胞易于向癌细胞转化.

Study on the relationship of the HP and expression of p53 gene in the patients with carcinoma and precarcinoma of stomach

Objective To study the relationship of the infection of helicobacter pylor(HP) and the expression of p53 gene in the patients with carcinoma and precarcinoma of stomach, and the mechanism of infection of HP on the carcinoma of stomach. Methods Using immu-nohistochemical S-P and toluidine blue chemic dying methods. Results The positive rate (65%) of HP infection in carcinoma is signifi-catively higher than that (32.5%) in chronic gastritis (P <0.01) , positive rate (73.1%) of p53 expression in the HP positive group is higher significatively than that (50%) in HP negative group ( P < 0.05) . The positive rate of p53 ex-preesion in the HP positive group is related to the degrees of differentiation soakage and the transplantation to lymph tissue. Conclusion The infection of HP is related to the carcinoma and precarcinoma of stomach and enhances mutation of p53 gene, which plays an important role in the carcinoma of stomach.
