内镜下对结直肠腺瘤性息肉切除的临床研究

摘    要:目的通过共聚焦激光显微内镜分析结直肠息肉的镜下组织学特点,探讨结直肠腺瘤性息肉切除的临床价值。方法 2007年6月至2010年6月间共计120例患者纳入本研究,均被病理证实为结直肠息肉。实验和对照组各60例。对实验组结直肠息肉进行息肉切除,评价息肉切除的临床价值。结果在两年内随访中,实验组发现结直肠癌3例,而对照组60例中,26例发生结直肠癌,阳性率为43.3%。结论共聚焦激光显微内镜在结直肠腺瘤性息肉诊断中精度较高,与组织病理学诊断具有较好的一致性,息肉电切除可预防结直肠癌变。

Abstract:Objective To analyze endoscopic histological characters of colorectal polyps with confocal laser endomicroscopy(CLE) examination and to explore value of polyp resection in adenomatous polyps.Methods From June 2007 to June 2010,120 patients were recruited,which pathologically confirmed colorectal polyps,60 for experiment group and 60 for control group.The patients in experiment group underwent polyp resection.At the same time,value of polyp resection was evaluated.Results In the follow two years,in the experiment group,there was no colorectal cancer.While in the control group,there were 26 colorectal cancer,and the positive rate was 43.3%.Conclusion The accuracy of CLE in adenomatous polyps is high,and the coherence with histopathology diagnosis is good.Polyp resection can prevent colorectal cancer.
