早期应用无创正压通气在急性左心衰抢救中的价值

目的：探讨无创正压通气在急性左心衰竭抢救中的价值及安全性.方法：将82例急性左心功能衰竭患者随机分为常规治疗组（41例）和无创通气组（41例）.常规治疗组给予吸氧及药物治疗;无创正压通气组在常规药物治疗同时给予无创正压通气,观察治疗前及后1、2h的血压、心率、呼吸频率及血气分析等临床指标的变化情况.结果：除血压外,两组治疗后的血压、心率、呼吸频率及血气分析等临床指标均较治疗前有明显改善,其差异有显著性（P＜0.05）,无创正压通气组抢救成功率90.2％,常规治疗组抢救成功率73.2％.结论：早期无创正压通气治疗急性左心衰时能迅速改善患者症状和低氧血症,改善心肺功能是一种快速安全有效的抢救方法.

Value of The Early Application of Noninvasive Positive Pressureventilation in the Rescue of Acute Left Heart Failure

Objective：To probe into the value and safety of the non- invasive positive pressure ventilation （NIPPV） in therescue of acute left heart failure（ALHF）. Methods： A total of 82 cases patients with acute left heart failure were at randomdivided into regular treatment group（41 cases） and noninvasive ventilation group （41 cases）. The oxygen inspirationand drug treatment were carried out in the regular treatment group, while the regular drug treatment plus noninvasive positive pressure ventilation were performed in the noninvasive ventila- tion group. The changes of the clinical indexeslike blood pressure, heart rate, breathing rate and blood - gas analysis on 1 hour or 2 hours before or after the treatmentwere respectively observed. Results： The clinical indexes in two groups like heart rate, breathingrate and blood - gas analysis obviously improved comparing with those before the treatment. The differences （exceptthe blood pressure） between the non - invasive ventilation group and the regular treatment group were obvious（P 〈 0.05）. In the noninvasive ventilation group, Success rate is 92. 5 %. In the regular treatment group, Success rate is 75 %. Conclusion： For the treatment of of patients with acute left heart failure, NIPPV shows the advantages of high success rate and being readily available. Such approach is worth spreading.
