替罗非班治疗高危组不稳定心绞痛的临床观察

目的：研究高危组不稳定心绞痛（UAP）患者使用替罗非班的治疗效果及安全性.方法：给予70例高危组不稳定心绞痛患者标准化的低分子肝素抗凝和阿司匹林、氯吡格雷抗血小板基础治疗,将患者随机分为对照组和治疗组.治疗组在基础治疗上增加替罗非班,观察在住院2周内两组患者心绞痛发作次数、心肌梗死发生率、出血不良事件发生和心电图的改善情况.结果：相比对照组,治疗组心绞痛发作次数和心肌梗死发生率显著减少（P＜0.05）.两组心电图的改善和出血不良事件发生率的差异无统计学意义（P＞0.05）.结论：替罗非班可有效缓解高危组不稳定型心绞痛的症状,减少心肌梗死发生率,有助于压低的ST段及T波倒置恢复,治疗期间未发生严重的出血倾向,临床应用安全有效.

Clinical Observation of High Risk Unstable Angina Pectoris Treated by Tirofiban

Objective： Study of the clinical curative effect and safety of Tirofiban Hydrochloride in the treatment of high- risk group of patients with unstable angina pectoris（UAP）. Method：Firstly, 70 high- risk patients with UAP were given standardization of aspirin and clopidogrel antiplatelet treatment as well as low molecular weight heparin treatment. Secondly, after previous treatments, patients still with UAP were divided into treatment group and control group based on the method of random digits table. Thirdly, the treatment group were given the Tirofiban Hydrochloride separately and the two groups were observed for two weeks on matters like the frequency of angina attacks, the incidence of myocardial infarction, the condition of electrocardiogram improvement and the occurrence of adverse bleeding. Results：Compared with the control group, the frequency of angina attacks and the incidence of myocardial infarction of the treatment group were decreased significantly（P 〈 0.05）. The electrocardiogram of the treatment group improved. There was no significant difference between the two groups（P 〉 0.05）. There was no significant difference between the two groups considering the occurrence of adverse bleeding（P 〉 0.05）. Conclusion： Tirofiban Hydrochloride is effective in easing pains, reducing the incidence of myocardial infarction and improve the electrocardiogram for patients with unstable angina pectoris（UAP）. Besides, during the treatment, there is no adverse bleeding. All in all, the clinical application of Tirofiban Hydrochloride is safe and effective.
