盐酸小檗碱对大鼠非酒精性脂肪性肝病的干预作用

摘    要:目的探讨盐酸小檗碱对大鼠非酒精性脂肪性肝病(NAFLD)的治疗作用。方法雄性SD大鼠55只,随机分为对照组(16只)普通饮食,造模组(39只)高脂饮食。8周后两组各随机抽取3只大鼠处死,证实脂肪肝造模成功后,造模组再分为高脂组、低剂量药物组和高剂量药物组各12只,分别给予普通生理盐水、小檗碱150 mg/(kg.d)和250 mg/(kg.d)灌胃。继续喂养4周后处死所有大鼠,留取血清和肝组织标本分别行常规生化检查及组织病理学NAS评分,免疫组化和PCR法检测肝组织过氧化物酶体增殖物激活受体-γ(peroxisome proliferatoractivated receptor-γ,PPAR-γ)蛋白和mRNA的表达。结果高脂饮食可以成功地复制NAFLD的大鼠模型;盐酸小檗碱组大鼠的脂肪变性和炎症程度较高脂组减轻,免疫组化PPAR-γ表达量下降;血清GLU、TC、TG、HDL-C、LDL-C在造模组表达量较正常组明显升高,差异具有统计学意义(P〈0.05);小檗碱组各值的表达量较高脂组改变显著,差异具有统计学意义(P〈0.01);PCR显示:高脂组PPAR-γmRNA表达量较对照组升高,应用盐酸小檗碱治疗后表达下降。结论盐酸小檗碱可以改善非酒精性脂肪性肝病的血脂紊乱及糖代谢异常,可能是通过降低PPAR-γmRNA表达来实现的;盐酸小檗碱有抑制肝细胞成脂性改变的作用;盐酸小檗碱可改善肝脏炎症,但对纤维化的作用尚需进一步研究。
