肝门胆管癌的CT诊断与分析

目的探讨肝门胆管癌的CT影像表现和诊断价值。方法搜集16例CT平扫加增强后经手术和病理证实的肝门胆管癌,分析肝门胆管癌的CT表现特征和组织病理学的关系。结果9例平扫表现肝门区低密度软组织肿块;7例表现为肝门区胆管壁不规则增厚,管腔狭窄;动态增强12例有延迟强化。对照肿瘤的组织病理学特点,评价CT扫描诊断肝门胆管癌的价值。结论肝门胆管癌的CT表现特征是肝门区低密度肿块和肝门胆管壁不规则增厚,伴肝内胆管扩张。CT在判断肿瘤浸润范围虽有一定局限性,但目前仍为诊断肝门胆管癌的一个重要检查手段。

CT diagnosis and analysis of hepatic hilar cholangiocarcinoma

Objective To study CT appearances and diagnostic value of hepatic hilar cholangiocarcioma. Methods 16 cases of hepatic hilar cholangiocarcioma by CT scan proved by surgically and pathologically were collected. The relation of CT characters with pathology were analyzed. Results On plain scan, it appeared like soft tissue mass with low density in hepatic hilar(n=9), bile duct wall thickened irregularly and bile duct narrowed (n=7). On dynamic enhanced scan,delayed enhancement was seen in 12 cases. Contrast to neoplastic pathological characters, diagnostic value of CT scan in hepatic hilar cholangiocarcinoma was studied. Couclusions CT characters of hepatic hilar cholangiocarcinoma are low density mass and irregalarly thickened bile duct wall in hepatic hilar with dilation of intra-hepatic bile duct. It has limitation in determining the invasive range of tumor, but CT is still an important procedure in diagnosirg hepatic hilar cholangiocarcinoma.
