摘    要:目的检测妊娠输卵管粘膜上皮及肌层雌、孕激素受体(ER、PR)的表达,以探索此两种受体和原因不明异位妊娠的发生之间的关系。方法确诊的输卵管妊娠(EP)病例38例,依据盆腔情况分为A组(无粘连)、B组(有粘连)。另选处于分泌晚期行其它妇科手术的的病例作为对照组15例。检测输卵管组织中ER、PR的表达。结果输卵管上皮及肌层ER在EP组较在对照组的表达为弱。输卵管上皮PR在三组卵管粘膜表达无显著性差异。肌层PR表达提示研究组A含量较B组及对照组均为弱。结论EP病人输卵管局部ER、肌层PR含量低,输卵管上皮发育及肌层蠕动功能与生殖活动不相适应,可能是EP发生的因素。应用米非司酮可作为早期EP的药物治疗手段之一。

Abstract:Objective: Observe quantitatively the expression of estrogen receptor(ER) and progesterone receptor(PR) in epithelium mucosae and muscular layer of pregnant oviduct, in order to investigate the etiologic relationship of these two receptor and tubal pregnancy without reasons. Methods: Detect the expression of tubal ER and PR in 38 cases of tubal pregnancy who have been confirmed and performed laparoscopy , including 20 cases without pelvic inflammation and adhesion (group A) and 18 cases with pelvic inflammation or adhesion(group B). 15 cases of patients in late secretion stage who were operated due to other gynecologic diseases were observed as control group. Results: The ER expressionsin in tubal epithelium and muscular layer are higher in control group. There is no statistical significance of epithelium PR among three groups. PR content in tubal muscular layer of group A is less than control group. Conclusion: Less ER content in epithelium and muscular layer of oviduct would cause low regional estrogen, and low PR content in muscular layer of oviduct, would lead to the inadaptation of tubal development and peristalsis to the needs of reproduction, which would result in the occurrence of EP. Mifepristone may be used as one of the drug treatment in early ectopic pregnancy.
