CT引导下经皮肺肿物穿刺活检安全准确性分析

目的观察在CT引导下经皮肺穿刺活检的安全性及诊断准确性。方法对29例肺部肿物患者在CT引导下行穿刺活检,将所取组织标本放入福尔马林中送检,另外将针槽中残余物涂片,95%酒精固定,送细胞学检查。结果29例病例穿刺均获成功,一次穿刺到肿块部位成功率为93.10%(27/29),二次穿刺均成功。穿刺后组织病检为肺部恶性肿瘤86.20%(25/29)、隐球菌病6.89%(2/29)、肺结核3.44%(1/29),无明确诊断,术后诊断为肺鳞癌并坏死1例3.44%(1/29),肺穿刺确诊率96.55%(28/29)。5例出现气胸,程度较轻,自行吸收,2例咯血,给予对症处理后好转。无其他并发症。结论CT引导下经皮肺肿物穿刺活检安全可靠,诊断准确率高,并发症少。

Analysis of Diagnostic Accuracy and Safety of CT-guided Percutaneous Transthoracic Lung Biopsy for Pulmonary Lesions

Objective To evaluate the diagnostic accuracy and safety of CT-guided percutaneous transthoracic lung biopsy for pulmonary lesions. Methods Total of 29 lung lesions were pathologically biopsy diagnosed through CT-guided fine needle aspiration. The tissues, which were immobilized by formalin, were diagnosed through pathologically biopsy. In addition, the relicts of needle slot, which were immobilized by 95% alcohol, were diagnosed through cytological examine. Results All 29 lung lesions were successfully biopsied. 27 of all 29 lung lesions were successfully biopsied at first time. Only 2 cases were successfully biopsied at second time. There were 25 cases were lung cancer, 2 cases were pulmonary cryptococcosis and 1 case was TB （pulmonary tuberculosis）. 1 case was diagnosed lung squamous cell cancer combining necrosis after the surgery. The diagnostic accuracy was 96.55%. 5 cases had gently pneumothorax and 2 cases had gently hemoptysis after the biopsy. Conclusion CT-guided percutaneous transthoracic lung biopsy has a high diagnostic accuracy, safe and little combining syndrome.
