老年食管癌贲门癌术后并急性心肌梗塞的临床分析

对１１１５例手术治疗老年食管癌贲门癌术后并发急性心肌梗塞（ＡＭＩ）４例分析，结果表明老年人心脏代偿功能减退及胸部大手术创伤的应激反应是术后并发ＡＭＩ的重要因素，无典型心绞痛及心律失常并发症率高是老年ＡＭＩ的特点。选择合适病例，做好术前准备及加强术后心脏功能监护是预防术后ＡＭＩ及降低老年人开胸术后死亡率的重要措施。

Clinical Analysis of Old Esophageal or Cardiac Carcinoma Complicated with Acuts Myocardial Infarction Following Operation

From Jan,1980 to Dec.1994,operations on 1115 cases of old esophageal or cardiac carcinoma were performed in our hospital.4 cases were complicated with acute myocardial infarction following operations.The analysis indicates that decreasing of cardiac compensation and stress reaction against damage of thoracic operation were the important factors of the post operative complications with the acute myocardial infarction. Non-typical stenocardia and the hight rate of complication with arrhythmia were the characteristics of the old acute myocardial infarction.Proper choice of patients,good preoperative preparation and (enhancing) adequate postoperative care of heart function were the important measures to prevent the postoperative acute myocardial infarction and to reduce the postoperative mortality of the thoracotomy in the old.
