右束支传导阻滞在急性心肌梗死中的意义

目的：评价右束支传导阻滞在急性心肌梗死中对临床和预后的影响。方法：对144例初发Q波性急性心肌梗死患者的病例资料做回顾分析,根据心电图结果将其分为两组,17例伴右束支传导阻滞的患者为观察组,对照组为同期收治的127例不伴右束支传导阻滞者,分析患者的基线资料、梗死部位、临床过程和住院病死率。结果：观察组患者休克、心力衰竭、恶性室性心律失常、前壁心肌梗死的发生率明显高于对照组（P〈0.05）,住院病死率观察组高于对照组（P〈0.05）。结论：本研究显示,急性心肌梗死特别是前壁心肌梗死伴右束支传导阻滞,临床经过不佳和预后差。

The significance of right bundle branch block in patients with acute myocardial infarction

Objective：To evaluate the influence of right bundle branch block（RBBB） on the clinical and prognosis of pa tients with acute myocardial infarction（AMI）.Methods：A total of 144 hospitalized patients with first Q wave AMI were analyzed retrospectively.Patients were devided into two groups based on the ECG findings,17 patients with RBBB were stud ied as observed group,the other 127 patients admitted in the same period without RBBB were studied as control group.The baseline characteristic,farction location,clinical course and in-hospital death were analyzed.Results：In observed group the rate of shock,heart failure,serious ventricular arrhythmias and anterior MI was much higher than those in the control group.In-hospital mortality in observed group was significantly higher when compared with control group（P〈0.05）.Conclusion：This study indicates RBBB with AMI,especially anterior MI is associated with bad clinical course and poor prognosis.
