川芎嗪合硫酸镁治疗妊娠高血压综合征的研究

目的　探讨川芎嗪合硫酸镁治疗妊娠高血压综合征 (妊高征 )的疗效 ,并通过观察胎盘结构的变化探讨其疗效机制。方法　 6 6例病例随机分为对照组 34例 ,用 2 5 %硫酸镁肌肉注射或 /和静脉滴注 ;治疗组 32例 ,加用盐酸川芎嗪静脉滴注。治疗后采集胎盘组织 ,进行病理学检查及透射电镜观察超微结构的变化。结果　治疗组较对照组能显著降低妊高征评分指数及平均动脉压 (P均 <0 .0 0 1) ,能提高降压及降蛋白尿疗效 (P <0 .0 5 ) ,妊高征患者的胎盘合体细胞结节增多、合体滋养细胞退变。治疗组合体滋养细胞胞浆空泡减少、表面微绒毛增加 ,毛细血管内皮细胞细胞器基本恢复正常。结论　川芎嗪合硫酸镁治疗妊高征能使胎盘血供在一定程度上有所改善 ,为有效、安全的治疗方法。

Study of ligustrazine combined with bitter salt on pregnancy induced hypertension syndrome

Objective It is to discuss the effect of ligustrazine combined with bitter salt on pregnancy induced hypertension syndrome (PIH) and the treatment mechanism through observing the changes of placental structure.Methods Sixty-six patients were randomly divided into control group (34 patients) and treatment group (32 patients). The patients in control group were injected or intravenous dripped with 25% bitter salt, the patients in treatment group were added intravenous dripped with ligustrazine. After treatment, the placental tissues were gathered and made pathology observation, and the changes of ultrastructure were observed with transmission electromicroscopy.Results The exponent scores of PIH and average artery pressure in treatment group were significantly lower than that in control group (P<0.001), and the effect of depressing blood pressure and albuminuria was heightened. The syncytiotrophoblast cell tubercles of PIH placenta were increased, the syncytiotrophoblast cells faded. In treatment group, the vacuoles in syncytiotrophoblast cell cytoplasms were reduced, the surface microvilli were increased and the endodermis cell organelles of the capillary were recovered.Conclusion Ligustrazine combined with bitter salt can improve the blood-supply of the placenta on some degree, so it is an effective and safe method on PIH.
