摘    要:目的比较缬沙坦、氨氯地平、美托洛尔3种降压药对原发性高血压患者血压变异性(BPV)的影响。方法湖北省大7台市中医院急诊科2010年1月-2013年6月收治的原发性高血压患者150例。随机分为缬沙坦组、氨氯地平组、美托洛尔组,每组各50例。动态血压监测(ABPM)收集血压和BPV数据。结果缬沙坦、氨氯地平可以显著降低大多数时段的BPV,缬沙坦降幅最明显。氨氯地平能降低日间舒张压BPV,优于其他药物。美托洛尔只能降低夜间BPV,对其余时段反而有升高作用。结论理想的降压方案需要关注BPV的影响。缬沙坦和氨氯地平能显著改善高血压患者的BPV。

Abstract:Objectives To compare the effects of three anti-hypertensive drugs-vatsartan, amlodipine and meto- prolol on blood pressure variability (BPV) in patients with essential hypertension. Method 150 cases of pa- tients with essential hypertension received in the Emergency Department of Daye Hospital of Traditional Chi- nese Medicine from January 2010 to June 2013 were randomly divided into valsartan group, amlodipine group and metoprolol group with 50 cases in each group. Blood pressure and BPV data were collected through ambu- latory blood pressure monitoring (ABPM). Conclusion Valsartan and amlodipine significantly reduced BPV during most of the time and valsartan had the more obvious effect. Amlodipine better reduced daytime diastolic BPV of patients than other drugs. Metoprolol only reduced nighttime BPV and increased BPV during the rest of the time. Conclusion The ideal anti-hypertensive program need pay attention to the effect of BPV. Valsar- tan and amlodipine can significantly improve the BPV of patients with hypertension.
