氟伐他汀治疗慢性充血性心力衰竭患者的疗效评价

目的：评价氟伐他汀治疗慢性充血性心力衰竭（CHF）患者的疗效。方法：63例CHF患者在常规抗心衰治疗基础上被随机分成氟伐他汀组（氟伐他汀40mg，每晚1次，n=31）和对照组（n=32），共治疗6个月。观察治疗前后血脂和高敏C反应蛋白（hs—CRP）的水平变化，以及左室射血分数（LVEF）、左室舒张末期直径（LVEDD）和心功能分级的变化；统计再住院率和病死率。结果：（1）治疗后，氟伐他汀组总胆固醇、低密度脂蛋白胆固醇和hs-CRP明显下降（P均〈0．05），对照组上述指标无明显改变。（2）治疗后两组的LVEDD和心功能分级均有下降（P均〈0．05），LVEF均有增加（P均〈0．05），但氟伐他汀组的心功能分级和LVEF改善较对照组更明显（P均〈0．05）。（3）氟伐他汀．组的再住院率低于对照组（P〈0．05）。结论：氟伐他汀可有效改善CHF患者的心功能，降低再住院率。

Therapeutic Effects of Fluvastatin in Patients with Congestive Heart Failure

Objective ：To evaluate the therapeutic effects of fluvastatin in patients with congestive heart failure （CHF）. Methods：Sixty -three patients of CHF with conventional therapy were randomized to fluvastatin group （fluvastatin 40mg, one time quaque nocte, n = 31 ） or control group （ n = 32） for six months. The changes of plasma levels of total cholesterol （ TC ）, low density lipopretein cholesterol（ LDL- C）, high density lipopmtein cholesterol（ HDL- C）and high sensitivity C reactive protein（ hs -CRP） ,left ventricular ejection fraction（LVEF） ,left ventricular end diastolic diameter（LVEDD） ,NYHA classification were determined. The rates for rehospitalization and mortality were observed. Results： （ 1 ） After treatment, the serum concentrations of TC, LDL - C and hs - CRP in the fluvastatin group were significantly reduced （ P 〈 0. 05 ）. There were no markedly change in the control group about above- mentioned indexes. （2）LVEDD and NYHA classification decreased significantly, and LVEF increased markedly in both groups after treatment （ all P 〈 0.05 ）. However, the changes of NYHA classification and LVEF were more significant in the fluvastatin group than those in the control group（ all P 〈 0. 05）. （3）The rate of rehospitalization was lower in the fluvastatin group than that in the control group（ P 〈 0.05 ）. Conclusion：These results suggest that Fluvastatin could improve cardiac function and decrease the rate of rehospitalization in patients with CHF.
