不同手术方式治疗开放性胫骨骨折疗效观察

目的探讨各类手术固定方式治疗开放性胫骨骨折的疗效。方法对1997年以来手术治疗的229例开放性胫骨骨折进行回顾性分析,其中钢板固定组55例,单侧外固定支架固定组89例,扩髓的加锁髓内钉固定组42例,不扩髓的加锁髓内钉固定组43例。结果随访0·5～10年,采用Johner-Wruh评分,总优良率为84·3%,加锁髓内钉治疗优于其他手术内固定,但扩髓与不扩髓之间差异无统计学意义。结论加锁髓内钉治疗开放性胫骨骨折Ⅰ～ⅢA型疗效可靠,值得在临床上推广。

Observation of the operative effect in the treatment of open fractures of the tibia shaft

Objective To analyse the effect of operative methods in the treatment of open fractures of the tibia shaft.Methods Two hundred and twenty-nine cases with operating treatment from 1997 were reviewed.There were 143 males and 86 females.All cases were commented by fracture type and operative methods.Results All cases were followed up from 0.5 to 10 years and they were evaluated according to Johner and Wruth.The results showed the rate of the excellence and good was 84.3%.The intramedullary interlocking nail is better than the other methods and there was no difference between the interlocking nails with and without reaming.Conclusion We can spreadly use intramedullary interlocking nail to treat tibia fractures from type Ⅰ to ⅢA for the good clinical treatment results.
