长期血液透析患者继发性甲状旁腺机能亢进症与肾性骨营养不良相关因素探讨

摘    要:将60例慢性肾功能衰竭(CRF)行血液透析(HD)患者分式4个组进行临床研究,40例HD5年以上,20例HD2年以下,各分别服用及不服用罗钙全治疗,分别测定甲状旁腺素(PTH)、降钙素(CT)、碱性磷酸酶(AKP)并进行X线骨照片。结果显示:全部患者PTH、CT明显升高,HD5年以上者高于2年以下者,不用罗钙全治疗者高于用罗钙全治疗者。HD5年以上不用罗钙全治疗者有95%肾性骨营养不良(ROD)、用罗钙全治疗者65%有ROD、透析时间越长,ROD发生率越高。

Abstract:he authors classified sixty chronic renal failure(CRF)patients receiving hemodialysis (HD)treat-ment into four groups for clinical study. Forty patients received HD for more than five years and the re-maining twenty patients received HD for less than two years,These two groups were further divided into two subgroups according to whether they took Rocaltrol or not. The levels of parathyroid hormone(PTH), calcitonic(CT), alkaline phosphatase(AKP) and bone X-ray were studied in each patient.The results showed:the levels of PTH and CT were obviously increased in alI the patients.The levels of PTH and CT were higher in the patients having received HD for more than five years than those for less than two years. They were also higher in patients who had not taken Rocaltrol than those who had. Ninety-five percent of the patients having received HD for more than five years had renal osteodystrophy(ROD)with-out receiving Rocaltrol treatment,while only sixty-five percent of the patients developed ROD with Roca-trol treatment.The longer the HD time, the higher the ROD incidence.The cause,prevention and treat-ment of ROD were discussed.
