云南瑞丽口岸入境驾驶员中HIV 1基因型分布与新重组型发现

摘    要:摘要:目的 了解云南省瑞丽口岸入境驾驶员主要流行HIV 1基因型的分布情况。方法 对云南瑞丽25
例HIV 1抗体阳性的入境驾驶员血清样本,用RT NestedPCR 方法扩增HIV 1gag (1155bp)、gag prot
(885bp)、RT (1010bp)基因片段,经核酸序列测定与拼接后,进行同源关系分析,确定其基因型,并进
行重组型分析并确定重组位点。结果 在本研究的25 例样本中,5 例PCR 扩增为阴性,未能有效分型。
20例确定基因型的样本中,CRF01 _AE 比例最高(40.00%), 其次为C 亚型(25.00%)、B 亚型
(5.00%)、CRF07_BC (5.00%)和CRF08_BC (5.00%),还有4 份为未知重组型(20.00%)。重组位
点分析发现,这4例未知重组是以CRF01_AE 为骨架,嵌入部分B 或C 亚型的基因片段,是在01_AE
重组的基础上发生的二代重组。结论 瑞丽口岸入境驾驶员中流行的HIV 1以CRF01_AE 为主,C、B亚
型及CRF07_BC/CRF08_BC 同时存在,并出现以CRF01_AE 为母株嵌入其他毒株基因片段的两种新型
重组,该人群中HIV 1基因型分布的复杂性正影响我国云南地区艾滋病分子流行病学特性。

HIV-1 genotype analysis among cross-border drivers going through Ruili port of Yunnan

Abstract:Objective To understand the distribution of HIV-1genotype among cross-border drivers going through Ruili port of Yunnan. Methods 25HIV-1positive serum samples were collected from truck drivers who entered mainland China through Ruili port of Yunnan in 2011.HIV-1RNA was extracted and gag(1 155 bp),gag-prot(885bp)and RT(1 010bp)gene fragments were amplified by RT-nested PCR and sequenced.Phylogenetic analysis was performed to determine the genotypes and the recombination sites were also identified for recombinants. Results Out of 25samples,20were successfully genotyped.CRF01_AE was found to be predominant(40%),followed by subtype C(25%),subtype B(5%),CRF07_BC(5%) and CRF08_BC(5%).The remaining 4samples(20%)were preliminary identified as novel recombinants, in which mosaic B or C fragments inserting in the CRF01_AE backbone.It was considered to be a novel second generation recombinant form of CRF01_AE. Conclusions CRF01_AE is a predominant genotype found among cross border truck drivers in Ruili port of Yunnan and novel second generation recombinant forms have also been identified in the study,indicating that the complexity of HIV-1genotype distribution contributes partly to the features of molecular epidemiology of AIDS in Yunnan.
