32例煤工尘肺肺心病合并冠心病诊断治疗体会

目的探讨煤工尘肺肺心病合并冠心病的发病特点和临床情况,减少临床误诊,漏诊。方法收集我院煤工尘肺肺心病合并冠心病32例的临床资料,对其诊治过程进行分析。结果本组病例经治疗好转28例,死亡4例:病因:心衰1例,心肌梗死、室颤1例,猝死1例,心衰合并呼衰1例。结论煤工尘肺肺心病合并冠心病患者临床病情复杂,常合并有多脏器功能改变,表现不典型,易发生急性或慢性左心功能不全。对于慢性肺心病并发冠心病的老年煤工尘肺患者,在诊治过程中应综合分析病情,以减少误诊、漏诊,使临床治疗发挥积极有效的作用。

Experience of diagnosis and treatment of 32 coal workers with pneumoconiosis pulmonary heart disease complicated with coronary heart disease

Objective To reduce misdiagnosis and missed diagnosis and to improve the living standards of patients by exploring the characteristics of pneumoconiosis pulmonary heart disease complicated with coronary heart disease.Methods The clinical data of 32 coal worker with pneumoconiosis pulmonary heart disease complicated with coronary heart disease were analyzed.Results In this group,28 of them recovered after medical treatment while 4 died.One died of heart failure,one of myocardial infarction and ventricular fibrillation,one of sudden death and one of heart failure complicated with respiratory failure.Conclusion The clinical conditions of patients with pneumoconiosis pulmonary heart disease complicated with coronary heart disease is complex,which are often combined with the changes of several visceral organs＇functions.Besides,the clinical conditions are atypical and prone to cause acute or chronic insufficiency of left heart.For the old coal workers with chronic pulmonary heart disease complicated with coronary heart disease,comprehensive analysis should be adopted when they are diagnosed and treated so as to reduce misdiagnoses and missed diagnosis and to ensure that the clinical treatment works effectively.
