固冲汤加减治疗无排卵型功血40例

目的:探讨中药固冲汤治疗无排卵型功血的机理。方法:70例无排卵型功血患者,随机分为中药治疗组和西药对照组,其中治疗组根据周期的不同阶段,采用固冲汤加减;对照组采用雌激素加安宫黄体酮,40岁以下者用克罗米芬促排卵;3个月经周期为1个疗程。观察其治疗前后的激素水平及内膜组织学变化情况。结果:中药组40例痊愈19例,有效17例,无效4例,总有效率90%;西药组30例,痊愈13例,有效11例,无效6例,总有效率80%(P>0.05)。中药组有25例进行了治疗前后的内膜组织学比较,有13例出现分泌期改变,且其激素水平无显著变化。结论:中药固冲汤加减有调节月经恢复排卵的功能,这一作用可能是通过提高垂体对LRH的反应性及卵巢对LH的反应性进而调节下丘脑-垂体-卵巢轴的功能而达到的。

40 Cases of Dysfunctional Uterine Bleeding of Anovulatory Cycles Treated by Guchongtang

Objective:To evaluate the clinical effect of "Guchongtang" on dysfunctional uterine bleeding(DUB)of anovulatory cycles and study the mechanism.Method:The study population made of 70 patients of DUB of anovulatory cycles,and they were devided into two groups at random:40 patients who were treated with "guchongtang"in group A and others with estrogen and progesterone in group B;meanwhile,all the patients under 40 years old received clomiphene citrate in group B.Their serum sex-hormone level and endometria tissue were observed.Results:In group A,there were 19 cases recovered and 17 got better,the totally effective rate was 90%;there were 13 cases recovered and 11 got better,the totally effective rate was 80% in group B;and there is no obvious difference between these two groups.The results also show that the endometria of 13 cases became secreting after treatment in group A,but serum sex-hormone level was same.Conclusion:"Guchongtang"could regulate menstruation and spur ovulation,the mechanism is that Chinese medicine can increase the reaction of pituitary to LRH and ovary to LH,all these can regulate the function of hypothalamic-pituitary-ovarian.
