多层螺旋CT对评价肝静脉流出道梗阻的价值

目的:探讨多层螺旋CT(MSCT)增强扫描及血管成像技术在诊断肝静脉流出道梗阻中的价值。方法:回顾性分析经MSCT扫描诊断为肝静脉流出道梗阻患者12例,其中肝移植术后流出道梗阻患者9例,布加综合征患者3例。其中5例患者行数字减影血管造影(DSA),5例患者接受剖腹探查手术或二次肝移植手术,均得到证实。结果:9例肝移植术后肝静脉流出道梗阻患者中,共发现6支肝右静脉闭塞,3支肝左静脉闭塞,3支肝中静脉闭塞,1支肝右后上静脉闭塞,4支肝后下腔静脉狭窄,2支肝后下腔静脉血栓形成。3例布加综合征患者均为混合型,肝静脉和下腔静脉同时受累。结论:MSCT血管成像作为一种无创性检查方法,在诊断肝脏流出道梗阻中具有重要意义。

Multislice CT in the evaluation of hepatic out-flow obstruction

Objective To explore the feasibility of the contrast enhanced CT scan and vascular imaging in the diagnosis of the hepatic out-flow obstruction. Methods Twelve patients diagnosed as the hepatic out-flow obstruction by contrast enhanced CT scan,including 9 with post-operative out-flow obstruction and 3 with Budd-Chiari syndrome,were reviewed. Five patients had the DSA results,and 5 patients underwent either exploratory or re-transplantation. Results Nineteen abnormal veins were observed in 9 patients with hepatic out-flow obstruction. All of the 3 patients with Budd-Chiari syndrome belonged to mixed type. Conclusions As a non-invasive technique,the contrast enhanced CT scan and vascular imaging may play an important role in the diagnosis of the hepatic out-flow obstruction.
