急性重度有机磷农药中毒并呼吸衰竭 53例救治体会

目的：探讨急性重度有机磷农药中毒（AOPP）合并呼吸衰竭救治方法。方法：53例在彻底洗胃和一般内科常规治疗基础上,采用突击量氯磷定疗法、尽早阿托品化、机械通气（MV）等综合抢救措施。结果：抢救成功44例,成功率为83%,自动放弃5例,死亡4例。结论：早期彻底洗胃、合理应用阿托品或盐酸戊乙奎醚、对昏迷患者先气管插管后胃管洗胃、早期、合理使用MV等救治方法配合恰当,可提高疗效。

Treatment of patients with severe organophosphorus pesticide intoxication complicated with respiratory failure:a report of 53 cases

Objective：To sum up the experience in rescuing patients with severe organophosphorus pesticide intoxication combined with respiratory failure.Methods：After thorough gastric lavage and general medical conventional therapy,53 patients with severe organophosphorus pesticide intoxication were treated by flushing dose of pralidoxime chloride,atropinization,mechanical ventilation,and other comprehensive emergency medical therapies.Results：Forty-four patients survived（83%）.Five cases gave up.Conclusions：Timely and thorough gastric lavage,proper application of atropine or changtuoling,rational use of tracheal intubation,mechanical ventilation and other techniques can improve the efficacy.
