胃肠手术患者尿管拔除时机的研究

目的比较胃肠道疾病开腹术后患者不同时间拔除导尿管后不良反应的发生情况。方法选择胃肠道疾病行开腹手术留置尿管的患者，分为观察组216例和对照组252例。观察组患者术后24h在膀胱充盈状态下拔除尿管，对照组按常规方法和时间拔除尿管。比较两组患者的主观感觉及拔除尿管后不良反应情况。结果观察组的尿路感染发生率为19．4％，尿潴留发生率为2．7％，术后排气时间（48．39±8．36）h，均优于对照组，差异有统计学意义（P〈0．01）；两组尿道出血及主观感受均无统计学意义（P〉0．05）。结论胃肠道手术患者术后24h在膀胱充盈状态下可拔除尿管，以减少尿路感染和尿潴留的发生，使排气时间更早。

The research of opportunity to remove urinary catheter in patients undergoing gastrointestinal operation

Objective To observe the adverse reaction after urinary catheter removal in different time in patient with gastrointestinal disease after lapratomy.Methods Selecting patients with gastrointestinal disease who need a urinary catheter after lapratomy,216 patients in intervention group and 252 patients in control group,patient in intervention group had the catheter removed 24h after operation when bladder was engorged,patient in control group adopted the routine method at routine time.Comparing the subject feeling and adverse reaction after catheter removal of patients in two groups.Results The incidence of urinary tract infection and urinary retention are 19.4% and 2.7% in intervention group,evacuating time after operation was (48.39±8.36) h,all the results surpass that of the control group,the differences were statistically significant (P < 0.05);there were no differences in urethremorrhage and subjective feelings (P > 0.05).Conclusions Patients with gastrointestinal disease can have urinary catheter removed 24 h after operation when bladder is engorged,which can reduce the incidence of urinary infection and urinary retention,shorten evacuating time.
