腹腔镜辅助脑积水分流术式的临床应用

摘    要:目的探讨应用一种新的脑室-腹腔分流方式技术。方法选取20例各种原因所致脑积水的病人,在脑室-腹腔分流手术中应用腹腔镜技术,在直视下将分流管腹腔端放置在指定位置,见脑脊液引流通畅后关腹。头部脑室穿刺手术和传统手术无异。结果术后住院期间,20例患者脑积水经CT或MRI复查全部缓解,脑室均缩小或恢复正常大小;出院后经门诊随访,除3例因分流管流速原因造成过度分流外,其余患者分流管均无阻塞现象。结论该手术方法简单、易行,可靠性强,手术时间短,腹部创口仅0.3~0.5cm,患者易接受,恢复快。

Abstract:Objective To investigate the application of a new V-P shunting manner. Methods Twenty patients with hydrocephalus caused by different reasons were selected. A new kind of operation technique, laparoscope-assisted V-P shunting, was applied. After traditional operative procedures of brain V-P, the shunting catheter of abdomen-portion was placed in the target position under direct vision of laparoscope. Results Observed by CT or MRI scan after operation, the hydrocephalus was relieved and the body of brain ventricle was reduced or recovered to normal size in all patients. The long-term follow-up showed that overshunting was seen in 3 cases and there was no any catheter obstraction in the rest. Conclusion The laparoscope-assisted hydrocephalus shunting is reliable, convenient with shorter operation time and the length of abdominal incision and quicker recovery.
