直肠内超声引导下经会阴前列腺穿刺活检的临床分析

目的探讨直肠超声引导下经会阴前列腺穿刺活检术在诊断前列腺癌中的应用价值。方法分析本院2011年1月—2013年6月收治的77例疑似前列腺占位病变患者,行直肠超声引导下经会阴前列腺穿刺活检,并与病理检测结果比较。结果 77例患者中,检出良性前列腺增生伴慢性炎症者占27.3%、前列腺不典型增生占10.4%、前列腺癌占62.3%。将患者按前列腺特异性抗原（PSA）值分为4组,4-10 ng/mL组、10-20 ng/mL组、20-50 ng/mL组、〉50 ng/mL组,各组前列腺癌检出率分别为20.0%（3/15）、43.8%（7/16）、71.4%（15/21）和92.0%（23/25）。与4-10 ng/mL组比较,其他各组前列腺癌检出率均显著提高（P〈0.05,P〈0.005,P〈0.005）。并发症发生率为11.7%（9/77）。结论直肠超声引导下经会阴前列腺穿刺活检术有助于前列腺疾病的诊断和鉴别诊断。

Clinical analysis of endorectal ultrasound-guided transperineal prostate biopsy

Objective To evaluate the value of endorectal ultrasound-guided transperineal prostate biopsy. Methods A total of 77 cases with suspected prostate diseases were subjected to endorectal ultrasound-guided transperineal prostate biopsy from January 2011 to June 2013. The results were compared with the final pathological findings. Results There were 21 cases（27.3%） of benign prostatic hyperplasia with chronic inflammation, 8 cases（10.4%） of atypical hyperplasia of prostate and 48 cases（62.3%） of prostate cancer found by pathology. According to the value of prostate specific antigen（PSA）, the cases were divided into four groups（4-10 ng/mL group, 10-20 ng/mL group, 20-50 ng/mL group, and 〉50 ng/mL group）. The prostate cancer detection rate was 20.0%（3/15）, 43.8%（7/16）, 71.4%（15/21） and 92.0%（23/25）, respectively. Compared with that in 4-10 ng/mL group, the detection rates in 10-20 ng/mL group, 20-50 ng/mL group and 〉50 ng/mL group were significantly increased（P〈0.05, P〈0.005, P〈0.005）. The incidence of complications was 11.7%（9/77）. Conclusion Endorectal ultrasoundguided transperineal prostate biopsy is helpful in clinical diagnosis and differential diagnosis of prostate diseases.
