自制弹性虹膜拉钩在小瞳孔白内障手术中的应用效果

目的探讨在基层医院运用更安全实用且价格低廉的自制弹性虹膜拉钩进行小瞳孔白内障手术的可行性。方法应用自制弹性虹膜拉钩对11例（12眼）因各种原因导致瞳孔不能散大直径小于4mm的白内障患眼行超声乳化和人工晶状体植入术。其中3例瞳孔机化膜较重，后借助维纳斯剪行放射状机化膜剪开。结果12眼均达到了边长为5～6mm正方形瞳孔，成功进行环形撕囊和超声乳化术，术后9眼获得近圆形瞳孔，无瞳孔撕裂。结论在小瞳孔晶状体超声乳化术中，自制弹性虹膜拉钩同样能获得良好的手术效果，但其具有易得、实用且费用低廉，适合在基层医院应用。

Effect of self-made inexpensive flexibleiris hook in smallpupils cataract surgery

Objective To investigate the feasibility of phaeoemulsification surgery in the setting of small nondilating pupils with a self - made, inexpensive flexible iris hook technique. Methods 11 cataract patients （ 12 eyes） who presented with nondilating pupils （less than 4 mm in diameter） were included. All the pupils were expanded with the assistance of self - made flexible iris hooks, phacoemulsification and intraocular lens implantation were performed. Radial cutting were underwent in the setting of dense fiber membrane with Viunas scissor in 3 cases. Results Pupils were extracted to square configure, and expanded to 5 or 6 mm in diameter, the prdcedure of continuous curvilineal capsulorhexis （CCC） and phacoemulsification showed uneventful in all cases. Conclusion The technique of self - made, flexible iris hook proved to be pleasant surgical outcome on phaco performed in grass - root hospital,with a feature of simple making, easy approach, inexpensive, as well as practical.
